Working within the philosophy of person-centred care, our complex dementia care services look to find individual strengths and needs. We support individuals in making their own decisions and choices regarding daily life and social activity wherever possible. Relationships are developed through a key worker approach that helps make every day as meaningful and enjoyable as possible. Care plans based on life histories and personal preferences are important, and involve collaborative work with relatives or friends.
Where it looks to be possible to resolve behavioural issues our multi-disciplinary team will work on helping the individual to overcome them.
Life skills are an important aspect of well-being and everyone is encouraged to manage their own care to the extent they can but our emphasis is on achieving the highest possible quality of life.
